First Visit

The first visit is a consultation where we can get to know you, examine your teeth and talk about different treatment options.  This visit is free of charge and you can request an appointment by calling, emailing, or using the request form below.  

What Should I Bring

There are a couple of forms available to the right – an adult and child acquaintance form.  Please print and fill out the appropriate form so we will have that information on hand.
